Practice Policies

Welcome to my Practice Policies page. I take the privacy and security of my patients very seriously, which is why I require consent when booking an appointment. On this page, I have provide a brief summary of some of my policies. You will be provided with more detailed policy information when you book your appointment, and you can request another copy at any time. Please be sure to take time to review the documentation. Consent for Treatment

All treatment is voluntary, and you may choose to stop treatment at any time. All new patients to undergo an initial evaluation for diagnosis and treatment recommendations. It's important to note that this evaluation serves as a consultation only and does not establish a therapeutic relationship. Once the evaluation is complete, we may come to a mutual agreement to establish a therapeutic relationship. In some cases, patients may decide to seek treatment elsewhere, or the physician may recommend another provider or treatment setting. In these cases, I will be happy to provide referrals to other mental health professionals. 

Appointments

At my clinic, I understand that each patient's situation is unique. Therefore, the frequency of appointments may vary depending on the your needs. Typically, patients have more frequent appointments at the beginning of treatment and then appointments are spaced further out as treatment progresses. To ensure the continuity of care, I strongly recommend scheduling appointments at least every 3 months. This will allow me to monitor your health and make any necessary adjustments to your treatment plan. My goal is to provide the best care possible for each patient.

Telehealth Appointments

Patients must be physically located in the states of either Missouri or Illinois at the time of their telehealth appointment, since I am currently licensed to practice medicine only in these states. I will ask you your location at the start of each appointment. If you are located outside of Missouri or Illinois, your appointment will be cancelled and you will be charged the rate for a missed appointment. Please see my payment and fees section for more information.

Written Communication

The majority of between-visit communication will be done through the patient portal. For emergencies in which you or someone else are in danger, please call 911 and go to the nearest emergency room.

Refill Requests

Refill requests must be made one week in advance. Appointments are required for controlled substance refills. Dose adjustments and medication changes will be done by appointment only.

Controlled Substances

Federal law places restrictions on prescribing controlled medications. One of these restrictions is that an in-person visit is required. Early refills for controlled substances are not allowed, including lost or stolen medications. Appointments are required for refills and dose adjustments. Patients taking controlled substances must be seen at a minimum of every 3 months.  Toxicology screens may be utilized during the course of your treatment. These policies are to ensure safety and well-being of patients, and to prevent misuse and abuse of these medications. It's important to follow these regulations to protect both patients and physicians.​

Payment and Fees

My appointment fees cover the time and expertise required to provide a thorough assessment and personalized plan for your needs. My fees are not conditional based on receiving a certain diagnosis or treatment. If you ultimately decide to pursue care elsewhere, fees are still required for the evaluation you received. Cancellations and Missed Appointments

Missed appointments will be charged the full fee to the credit card on file. This also includes a telehealth appointment in which the patient is physically located in a state other than Missouri or Illinois. Late arrivals are also charged at the rate of a full appointment. If you desire, we can meet for the remainder of scheduled time.

Confidentiality

Protected health information is kept private and stored on secure servers within my electronic medical record system. This information will not be shared with any third parties unless written permission is provided by the patient. Here are two examples of some of the very rare circumstances where the physician is required by law to share information: 1) If the physician believes the patient is in imminent danger of harming themselves or someone else and 2) If the physician believes a vulnerable person (e.g. child or elderly) is being subjected to abuse or neglect.
